The following are notices regarding professional conduct as required by the Health Professions Act and/or a Hearing Tribunal Order.

1. NOTICE OF HEARING TRIBUNAL ORDER:
By order of a Hearing Tribunal held in Edmonton on March 22, 2011 please be advised that Ms. Deborah Shirley Olga Kowlaski's Registration and Practice Permit has been cancelled by a previous Tribunal. As of March 22, 2011 there have been nine (9) Hearing Tribunal Hearings regarding Ms. Kowalski with multiple findings of unprofessional conduct. Ms. Kowalski may not provide any Denturist services, nor use the protected title of Denturists in the Province of Alberta. This notice is to remain posted until December 31, 2018.

2. NOTICE OF PERMANENT CANCELLATION OF REGISTRATION AND PRACTICE PERMIT:
The College of Alberta Denturists, the Regulatory Body for the Denturists Profession in Alberta pursuant to the Alberta Health Professions Act, is hereby providing notification of the Permanent Cancellation of a Practice Permit and Registration, for its Former Regulated Member, Ms. Deborah Shirley Kowalski.
Take note that Ms. Deborah Shirley Kowalski cannot legally provide any denturist services, and/or hold herself out to be a denturist, and/or use the protected titles of “denturist” and/or “regulated denturist”.
